How to Request Authorization for Out-Of-Network Services created by the State of Maryland
Services may be covered in full or in part by your health insurance or employee benefit plan. You may be able to use your HSA to cover the cost of your therapy. Please check your coverage carefully by asking the following questions:
- Do I have mental health benefits for Family counseling?
- What is my deductible and has it been met?
- How many sessions per calendar year does my plan cover?
- How much does my plan cover for an out-of-network provider?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- Do I have out-of-network benefits?
Reduced Fee
Reduced fee services are available on a limited basis.
Payment
Cash, check, master card, visa, and discover credit cards accepted for
payment.
You may also use your HSA or Flex Spending account.
Cancellation Policy
If you do not show up for your scheduled appointment, you will be
required to pay the full cost of the session in advance to reschedule.
The first late or for any reason cancel will have a charge a charge of $75.00, after this,
A ll cancelled appointments less than 48 hours are charged the FULL fee . This time has been set aside for you and is a lost
appointment time for your therapist.
